Answer
When the 'controlled upgrade' feature is used APs will not automatically upgrade their firmware to match the controller firmware. APs must be manually upgraded. When an image is applied to an AP through the Controlled Upgrade tab and the 'Save for later' option is used APs will upgrade if the 'Upgrade now' option is selected, the AP is power cycled, the AP is rebooted via AP CLI, the AP is rebooted via controller GUI/CLI.

controlled upgrade (no image applied to AP) 
-Issue reboot of AP from controller GUI/CLI - No upgrade 
-Power cycle AP - No upgrade 
-Reboot AP from AP CLI - No upgrade 

controlled upgrade (image applied to AP/saved for later) 
-Issue reboot of AP from controller GUI/CLI - AP upgrades 
-Power cycle AP - AP upgrades 
-Reboot AP from AP CLI - AP upgrades
