2.    Part Number of the unit (model)
3.   Shipping address where the new unit will be sent
4.   Contract name and phone number for RMA hardware delivery (receiving replacement product)
5.   Contact's email address (who will receive the new device) - for UPS delivery or other carrier (this information will appear on the shipping label).
6.   Problem description (reason for failure)
