TitleVSP 4850 Error On Console "root: Warning: Possible USB problem, rebooting in 2 minutes."
Symptoms

Virtual Services Platform 4000 running any supported software version.
Switch will not fully boot, or takes longer than expected.  Few VSP4850GTS may intermittently be affected by overclocking on the USB interface at the chip level which can lead to repeated reboots when being reset. During the repeated reboot, message 'Warning: Possible USB problem, rebooting in 2 minutes' is seen on the console output with the below dump:

Cause

Issue is due to a software defect of U-boot image.

 

Resolution

Issue is resolved in U-boot image software ""VU-Boot 2012.04-00001-g6a10a19 (Apr 18 2014 - 16:22:07) MPC83XX" which is available for download.
This is a result of an underlying issue with older U-Boot images (-> ‘U-Boot 2012.04 (Mar 26 2013 - 17:44:29)MPC83XX’).

There are two behaviors that couple with this error. 
Switch will eventually boot or the switch will not fully boot and proceeds to a boot loop.
Each behavior is approached differently.

Switch will not complete boot process:
Switch will need to set for a repair / replacement (RMA).
Login to your Extreme portal account and open a ticket with GTAC:

Switch will complete boot process:
U-Boot image will need to be upgraded.

Syntax:
Enter Privileged EXEC mode:
>enable

Upgrade the boot loader image:
#uboot-install WORD<1-99>

Example:
VSP4850GTS:1#uboot-install /intflash/20140418_4850GTS_uboot.tgz
Archive contain the following uboot build 20140418_4850GTS_uboot
UBOOT is out of date, requires an upgrade

NOTE: Important this is done as soon as possible because of the nature of the problem.
Switch may complete boot, but will eventually fail completely if U-Boot is not upgraded.
