Cause
One of the reason for this could be the process tftpd is not running.
The tftpd server process is used for internal file transfer between I/O modules and MSM, if this process is not running the to and fro file transfers will be affected.

 
Resolution
check if the process tftpd is in stopped state, if yes please use the command "start process tftpd" and verify if the process is in ready state and then try the "show version images" and I/O module install.
Switch # show process "tftpd"
