To request a replacement for your affected product, visit the Extreme Support Web site (http://www.extremenetworks.com/support/
), and login to the Portal to open a case to ‘Request a Hardware Replacement’. In the ‘Description’ field, please include ‘Field Notice-415’ to ensure proper processing.

Label is located on bottom of product.
The last two digits of the Shipping number are the Mfg Rev of the product. Mfg Rev 5J and earlier are affected
To Remotely determine product Hardware Version:
Log into the C35 Wireless Assistant. From the main page, browse to Reports -> Controller Status -> Manufacturing Information. The Hardware Revision is displayed as shown below.
Hardware Revision = 01 (corresponds to manufacturing revision 5J and earlier)
Hardware Revision = 02 (corresponds to manufacturing revision 5K and later)
