Symptoms
While rebooting a node in a stack, the other slots also reboot randomly. Following logs are seen in stack:

08/23/2016 18:33:34.31 <Warn:DM.Warning> Slot-7: Slot-2 FAILED (1)

08/23/2016 18:33:28.61 <Warn:DM.Warning> Slot-2: Slot-1 FAILED (1) Conduit receive error encountered
08/23/2016 18:33:28.20 <Erro:DM.Error> Slot-2: Shutting down Slot-1
08/23/2016 18:33:28.20 <Warn:DM.Warning> Slot-2: System Error 0: Conduit receive error encountered
08/23/2016 18:33:28.20 <Warn:HAL.Card.Warning> Slot-2: pibConduitMasterRcvOneSlot(): failed, rc=-6, errno=0, slot=1

Cause
The issue is seen only when the sys-recovery-level is configured to be shutdown instead of reset.

Sys-recovery-level options and their explanation

This is identified as a bug and the CR xos0066008 is created to fix this permanently
Resolution
Fix is available in the software versions:
  • 16.1.4.2-patch1-7 and above.
  • 21.1.3.2 and above
