When creating an Offense Rule, the Send to Forward Destination table is empty, despite having existing forwarding entries in the Manage Destinations admin window.
1. Offenses-->Rules-->Create new Offense rule
2. In the Rule Response window, check 'Send to Forward Destination'. This list will be empty, despite the fact that there are entries in the forwarding table if you click on 'manage destinations'.
3. Also in the Rule Response window, Select 'Response Limiter'. Either use the default settings or enter any value in either the 'times' or 'per' field, and an error will occur when you hit 'next'. Error is: 'Please enter a valid option for per index'
